Advance Care Planning is the process of meeting with a patient, their loved ones and a palliative medicine professional to:

1. Discuss values and goals related to quality of life and medical care.

2. Document those values and goals.

3. Plan medical care based on the patient's values and goals.


The process of Advance Care Planning has shown significant advantage to patients and their loved ones when compared to the more common practice of advance directives.
